Terms and Exclusions for Promotional Offer

Eligibility Criteria for Referral:

1. Residency Requirement: The referred individual must reside in one of the following states: Florida, Texas, Alabama, or New York.
2. Age Requirement: The referred individual must be between the ages of 18 and 64.
3. Income Requirement: To be potentially eligible for a low-cost or no-cost health plan, the referred individual’s annual household income should be under $50,000.

Conditions for Receiving the Referral Incentive:

4. Agent Consultation Requirement: To qualify for the referral incentive, the referred individual must anticipate being contacted by an agent appointed by the Health Exchange. The individual must be willing to set an appointment or dedicate at least 10 minutes to discuss health plan options with the agent.
5. Maximum Entries: You can submit unlimited referrals, but only up to 8 referrals per member are eligible for referral concession.

This gift card may be applied toward any purchase made at any participating location. This gift card is not exchangeable for cash or replaced if lost or stolen. After 2 years of non-use, a $2.00 per month fee will be charged
